Attention all students:

Health Services will transition to an electronic medical record on Tuesday, January 17, 2012. We ask that all students who need an appointment call 508-793-7467 to schedule this appointment. We will be unable to accommodate walk in appointments. As a result of this change, it is vital that you bring the following information to all visits:

Current Insurance card(s)

• A current list of medications including dosages and times
• A list of allergies
• The name and address of the pharmacy you wish to use and/or mail order pharmacy information, if you use one
• The name of the laboratory where you wish your lab work to be sent. We are currently using UMass Memorial and Quest Laboratories.

 If you are unsure of your benefits, you should call the number on your insurance card prior to your visit.
